Specimens held at room temperature show measurable photo-oxidation of bilirubin after four hours of ambient light exposure. Centrifuged, gel-separated tubes stored at 2–8 °C remain acceptable for up to 72 hours; whole blood does not. Laboratories reporting neonatal direct bilirubin should validate local stability limits against their transport conditions.
